NYC Hepatitis B Coalition: Needs Assessment Survey & Kick-Off Event
June 01, 2009
In preparation for the first NYC Hepatitis B Coalition meeting on June 19th, we are disseminating a brief Needs Assessment Survey in order to better understand gaps and challenges in the NYC hepatitis B service delivery system.
We need organizations in New York City who are working on hepatitis B or are interested in working on hepatitis B to fill out this survey!
It would be very much appreciated if you would:
* Complete this brief Hepatitis B NYC Needs Assessment Survey (click on the link below).
* Forward this Hepatitis B NYC Needs Assessment Survey to anyone else within your or outside your organization who works on (or is interested in) hepatitis B related issues. We are trying to distribute the survey as broadly as possible to get a good representation of viewpoints and experiences.
Please return this survey by June 15th, 2009
Sincerely,
The NYC Hepatitis B Coalition Planning Committee
